“One of the exciting things happening at St. Elizabeth that I am proud to be part of is the culture change where we have moved from an institutional model to an elder centered model of care. Some of the changes involve name changes. Our units are now called neighborhoods. This is to signal a profound change in our approach to care where our elders’ wants and needs are our driving consideration rather than on the activity schedules developed by the staff. This culture change has turned the whole organization around to focus on a person-centered approach. The managers of the neighborhoods are now Neighborhood Guides rather than Nurse Managers. This frees the nursing staff to concentrate on clinical activities, and the Guides can focus on all of the other activities of daily living. Another key ingredient in the culture change has been to involve the whole staff in meeting elders’ needs. Neighborhood Guides, Nurses, Geriatric Nursing Assistants, Culinary Team Members, Cooks, Housekeepers, Maintenance Staff and Recreation Associates are all members of the team. It has been a most interesting process for me to mentor the staff through this change. We are well along the way; however, the process is ongoing.”

What we offer
St. Elizabeth’s offers 24-hour skilled nursing care centered on the person in a setting that was designed to be the elder’s home while they are at St. Elizabeth. The goal of the skilled nursing program is to enhance the elder’s quality of life, while accommodating their preferences and daily routine. Skilled nursing care is provided by a team of specialist’s, eder associates, and nurses who are certified. The most important member of this team is the elder. Areas of specialized care within St. Elizabeth include memory care, long term care, and short term rehabilitation. Although the team is multidisciplinary, its focus is to work as a neighborhood meeting the primary needs of the elders at any given moment.
